Perkins was born on February 27, 1896, to Perry Perkins and Hannah Doran Perkins. He initially played second base for Gloucester High School but switched to catcher during a game in his sophomore year. One of his teammates was Al “Brick” McInnis, the younger brother of Stuffy McInnis, who was a member of the Philadelphia Athletics. Stuffy McInnis worked with the Gloucester High team and noticed Perkins. He recommended Perkins to the A’s, where he signed a professional contract before his senior year.

He played as a catcher in Major League Baseball, most notably for the Philadelphia Athletics. After his baseball career, he became a coach for the New York Yankees and the Philadelphia Phillies.
